| Inpatient Admissions
As a long-term acute care hospital, Gaylord Hospital specializes in the care and treatment of medically complex patients and rehabilitation. Long-term acute care (LTAC) is a recognized designation by the Centers for Medicare and Medicaid Services for acute care hospitals whose average length of stay is at least 25 days. LTAC hospitals provide specialized care services to manage medical conditions, so that patients with catastrophic or acute illnesses and injuries can work toward recovery. As an LTAC, Gaylord Hospital is staffed and equipped to handle the specific needs of acutely ill or chronically disabled patients who require a hospital level of care. Gaylord Hospital also is able to treat those who need rehabilitation for illnesses or injuries related to the brain and nervous system, bones and joints, lungs, and other conditions. Gaylord’s Rehabilitation Division offers services in: The Admissions Office works to ensure a smooth transition for patients moving from the acute care setting into Gaylord's inpatient programs. For more inpatient information, call the Admissions Office: (203) 284-2810. Referrals A patient is usually referred to Gaylord from an acute care hospital. A physician, social worker or discharge planner makes the referral by calling Gaylord's Admissions Office at (203) 284-2810. Occasionally, a representative of an insurance company initiates arrangements. The admission process is straightforward and usually can be accomplished within 24 hours. |
On-site Evaluations Gaylord's nurse-evaluator frequently visits a patient while he or she is still in the acute care hospital if the patient's condition permits such a visit. The nurse-evaluator describes Gaylord and its programs to ensure that the services Gaylord offers will match the patient's needs. Tours Structured tours are available to familiarize patients, their families and significant others with Gaylord and its services. Tours may be arranged by calling Nursing Administration at 1.866.GAYLORD, ext. 2815 or (203) 284-2815. Registration and Admission Upon admission to Gaylord, the patient and the patient's significant others are directed to the patient's assigned room. There, an admission interviewer verifies basic patient information such as the patient's name, address, social security number and insurance. Care Management Care managers ensure that each patient is admitted to and discharged from Gaylord at the most appropriate level of care. Upon admission, each patient is assigned a care manager who follows the patient through the full continuum of care. Outpatient Referrals and Appointments Gaylord offers outpatient services at its main campus in Wallingford and at locations in New Haven and Woodbridge. Insurance Verification and Financial Counseling The hospital reviews every potential inpatient and outpatient admission whether or not the patient is ever admitted to Gaylord to verify health insurance information and the level of the patient's health benefits. Gaylord also is responsible for informing the patient and family about the financial impact of treatment. Before treatment begins, Gaylord makes a good faith attempt to provide the patient with an accurate estimate of treatment costs that may not be covered by the patient's insurance.
