For Inpatient Referrals, call the Admissions Office at (203) 284-2810. Gaylord's regional health care managers will answer your questions and facilitate your patient admissions.

The Admissions Office will collect the necessary medical information and channel all paperwork to the appropriate sources. When required, the Admissions Office also will work with the discharge planner to arrange for a Gaylord registered nurse to conduct a physical evaluation of the patient being referred. The Admissions Office also will conduct telephone evaluations.

For Outpatient Referrals, call the Outpatient Department at (203) 284-2818.
