Make a Referral to Gaylord Specialty Healthcare

Hospital Referrals

To refer a patient from an acute care setting, call the Admissions Department at (203) 284-2810, use AllScripts, or FAX: (203) 284-2811. Gaylord's Admissions staff can answer questions and have a Clinical Evaluators, who is a registered nurse, review the patient's clinical needs, conduct a physical evaluation, and determine if the patient is appropriate for admission. An admissions nurse conducts chart reviews for patients located outside of our geographic area. 

Evaluation Criteria

    Does the patient have multisystem failures?

    Is the patient on a ventilator?
  • Gaylord-swirl-1
    Is the patient in need of intensive wound care?
  • Gaylord-swirl-1
    Is the patient too unstable for transfer to a lower level of care?
  • Gaylord-swirl-1
    Is the patient’s rehabilitation compromised by acute medical issues?
  • Gaylord-swirl-1
    Is the patient’s anticipated length of stay longer than 14 days?
  • Gaylord-swirl-1
    Is the patient in the intensive care unit?

If the answer to one or more of the evaluation questions is yes, the patient may be eligible for admission to Gaylord Hospital

Gaylord's in-house scheduling staff will assist in referring office staff or patients/families. A Regional Manager is available to visit community offices to discuss services or develop a referral process to best suit your needs. We can help your team expedite referral paperwork and provide patient education materials to assist in explaining why your patient is being referred to Gaylord for a specific service.

Traurig House - Transitional Living Center


Referrals for our residential living center for stroke and brain injury survivors come from acute care hospitals and rehabilitation settings. Contact Traurig House Nurse Care Manager, Amy Benoit, RN, to discuss your patient’s needs.

Gaylord Physical Therapy referrals may be directed to the contact information below. If there are questions regarding Gaylord Physical Therapy services, technologies, or appointments, please call the specific location that you have a question about. An Outpatient Regional Healthcare Manager is also available to visit referring healthcare providers to discuss our services. We can supply materials to expedite referral paperwork and provide patient education materials.
