Physician Appointments

Physician Services / Physiatry

Gaylord’s medical doctors are well-known for their decades of experience as physical medicine and rehabilitation doctors, also called physiatry. These specialists look at the whole person for solutions using non-surgical and non-narcotic rehabilitation-based relearning methods or finding modified ways to return to active life. Addressing musculoskeletal concerns from your shoulders to your ankles, including knee and elbow pain, this team gets people up and moving again.

How To Enroll

Outpatient Pulmonary Rehab

If your community doctor has diagnosed you with conditions such as COPD, long COVID, emphysema, or interstitial lung disease, outpatient pulmonary rehabilitation might be the right choice for you. Our 9-week program offers a comprehensive approach, providing education and exercise to enhance your understanding of your respiratory condition.

To take advantage of this program, follow these steps to schedule your outpatient pulmonary rehab:

Request a Referral: Ask your doctor for a referral to our outpatient pulmonary rehabilitation program. This referral is essential to initiate the process and ensure the program is tailored to your needs.
MD faxes history, diagnosis, medication list, pulmonary function test (if applicable) to (203) 294-8705
Our Pulmonary Team will schedule you to come in for evaluation and test prior to your formal enrollment.

Neuropsychological assessment is often needed when a medical condition or injury has affected a person’s memory or executive functioning skills.

Grieving the loss of one’s physical or mental capabilities following a life-changing event is a natural part of adjusting to life after a serious illness or injury. Counseling with a licensed clinical social worker can help you proceed through your healing journey.

A referral from your doctor is needed for both neuropsychology and counseling.
