Gaylord Hospital Admissions

The Admissions Process

1. Referral.
Patients admitted to Gaylord Hospital are usually referred from an acute care hospital by a physician, social worker, case manager or discharge planner who makes the referral by calling Gaylord Admissions at (203) 284-2810. Sometimes an insurance company representative initiates the arrangement. Hospital Referrals

2. Determination of medical necessity and appropriateness.
Once the referral has been made, a Regional Healthcare Manager from Gaylord Hospital visits the referring hospital, conducts an on-site evaluation and completes a preadmission assessment of the patient to ensure that Gaylord is the next most appropriate level of care. The assessment may include interviews with the patient, family, physicians and any other health care professionals directly involved in the patient’s care. We will then seek authorization from your insurance company for admission. 

We encourage patients’ family members to tour Gaylord Hospital prior to admission. Tours can be arranged by appointment on weekdays by calling: (203) 284-2800, option 9. 

3. Insurance coverage verification and financial assistance.
It is our responsibility to inform the patient and family about the financial cost of the patient’s hospital stay. Before admission, Gaylord makes a good faith attempt to provide the patient with an accurate estimate of treatment costs that may not be covered by the patient’s insurance. Gaylord does not discriminate on the basis of a person’s ability to pay. Before receiving services at Gaylord, we suggest you confirm coverage with your employer or private insurance carrier and to determine if your insurance is accepted by Gaylord. 

Insurance Information

Financial assistance may be available to patients who meet income guidelines and have been denied Medicaid benefits. Gaylord offers payment plans to those who don’t meet income guidelines or do not qualify for Medicaid. Payment plans may be discussed with our Billing Department at (203) 741-3368.

4. Admission to Gaylord Hospital.
A clinical admissions representative works with the referring hospital and physicians to arrange for the date and time of admission. The room and bed will be assigned based on clinical availability and bed availability.Upon arrival, an attending physician will be assigned to direct and oversee care. A care manager will be appointed. An admissions representative will help complete registration and verify that basic information is correct. A Notice of Privacy Practices that safeguards the patient's health information will be reviewed and signed.
